Surgery policy:

 I do not perform surgery (including dentals) with a small animal clinic on the first shift worked or until we are familiar with one another.  I do not perform cosmetic tail docks, ear crops, or dewclaws.

Cancellation policy:

You may cancel this contract no less than two weeks before the above date(s) or you may incur a late cancellation fee of 25% of the estimated total per full day scheduled. If more than 2 weeks notice of hour reduction is given, contracted hours on the affected days may be subject to cancellation if changes are not mutually agreeable.

Taxes:

Please provide me with a 1099 tax document.
